Admissions
Many personal care items such as mouthwash, tooth paste, shampoos, soaps, lotions, denture care items, ointments, undergarments and other items are provided. A list of items will be provided on admission.
No. Laundry services are part of our daily rate. Please make sure that your clothing is marked. Please ask staff for assistance if you need help marking your clothing.
The facility does not provide a phone in resident rooms. Many residents do use cell phones or will have a phone hooked up through the phone company. If you need to make a call, a phone and private space can be provided.
Bed, closet, and chest of drawers are provided. You may want to bring your own recliner and TV. There is free WiFi and cable services.
Pine Crest strives to maintain a home like environment. Visiting hours are open. The doors are locked during night time hours for resident and staff safety.
Bring clothing that is wash and wear and is comfortable for you to wear in your daily activities. You should bring at least seven sets.
Camping in Lincoln County

Overnight camping, including tents, trailers, cars and trucks may be permitted in the County Forest without charge for a period of no more than 14 days cumulatively in a calendar year. Any camper who violates the rules of good conduct, including cutting or defacing timber, carelessness with fire, violation of game and fishing laws of the State, or improper disposal of garbage and litter shall be subject to ejection from the County Forest and subject to the penalties provided by County and State laws.
When camping on the County Forest, blocking roads, parking areas, or lake access is not permitted.
Camping is not allowed at the following lakes:

Henson Lake
Kordick Lake
No, the campsites are not reservable.

Emergency Preparedness
Preparing a Family Emergency Plan is easy. You can get started by using the free Family Emergency Plan template. A Family Emergency Plan can help a family to stay in contact if they are separated during an emergency.
It's important that you know the potential emergencies that can happen in your state, region and community. Go to ReadyWisconsin.wi.gov and click on our ReadyWisconsin Facebook, Twitter, or Email buttons to get emergency information directly from Wisconsin Emergency Management. ReadyWisconsin also recommends including a battery operated radio, such as a NOAA radio with tone alert, in your emergency kit to stay apprised of developments during emergencies. It is always important to listen to and follow the advice of local officials in the event of an emergency.
For children under 10, ReadyWisconsin recommends "Let's Get Ready". Click here to watch this great video from the folks at Sesame Street.
For older kids, check out the STEP program. STEP is designed for teachers and others to get 4th and 5th graders ready for emergencies.
ReadyWisconsin encourages all individuals to make an Emergency Supply Kit, including food and water, medications, a list of medications, dosage and schedule and medical records, and personal items such as eyeglasses, hearing aids, wheelchair battery charger and other appropriate supplies to maintain health and independence. ReadyWisconsin also recommends that individuals develop an emergency plan that considers their unique needs and a personal support network they can call upon in the event of an emergency. Additionally, every individual should include their friends and neighbors, with disabilities and without, in their emergency planning. In an emergency, neighbors will need to be prepared to help each other until first responders arrive. Know if your neighbors have disabilities or other access and functional needs and help them prepare.
Pet owners should assemble an Emergency Supply Kit that includes enough pet food and water for three days, medications and medical records, leashes, ID tags, and other appropriate supplies. It's recommended that pet owners have an emergency plan that includes the needs of their pets. Taking into consideration the types of emergencies that could happen in their area and the appropriate responses.
In a basic Emergency Supply Kit, the ReadyWisconsin Campaign recommends including the following:
One gallon of water per person per day for three days – remember to include enough for your pets, too!
At least a three-day supply of non-perishable food. Select foods that require no refrigeration, preparation, or cooking and little or no water, and choose foods your family will eat: ready-to-eat canned meats, peanut butter, protein or fruit bars, dry cereal or granola. Also pack a manual can opener and eating utensils.
Battery-powered or hand crank radio and a NOAA Weather Radio with tone alert and extra batteries for both
Flashlight and extra batteries
First aid kit
Whistle to signal for help
Moist towelettes, garbage bags and plastic ties for personal sanitation
Wrench or pliers to turn off utilities

A Community Health Improvement Plan is a long-term, organized plan that will be used by a community to address identified public health priorities.
The purpose of an action plan is to outline the specific tasks that need to be completed to reach a goal. There are measurable goals identified for each CHIP priority. The goals are further broken down into objectives with associated actions steps or strategies of how to accomplish the objectives.
Each of the Health Priorities has an Action Plan that can be view at the bottom of its respective page.
An electronic copy for you to read or/and print can be found in both the “What’s New” and “General Resources” pages. You can also visit us at the Lincoln County Health Department for your own copy.
When the efforts of local and state health improvement plans are aligned, the impact of each individual plan and effort is greater than if they had completely different focuses.

Yes. The website indexes web pages as well as PDFs, Microsoft Office documents, and text documents.
No. By default, the search results will show matches for any word within the phrase. In this example, you would receive results for all web pages and documents that contained either the word alarm or the word permit or both.
In order to search on an exact phrase, enclose your search phrase in quotations. The search results for "alarm permit" will show matches for that exact phrase.
Yes. You can exclude words by using the minus sign (-). In order to find the results of all pages that have alarm in the result but not permit, you would search for alarm -permit.

The Sheriff's Office is not allowed to give legal advice of any kind. You may contact an attorney for legal advice.
No. The Sheriff's Office will only check papers to make sure they are legible and accurate.
2 copies are required with the official court stamp on them for each person to be served.
No. The Sheriff's Office will not provide an address. You will need a current physical address in order for us to attempt service.
We will attempt to accommodate this, but it can be very difficult to schedule an officer to be somewhere at a specific time.
Most forms are obtained from the Clerk of Courts Office located in the Courthouse. Most forms are free, but there are filing fees.
