1. What is a Community Health Improvement Plan (CHIP)?
A Community Health Improvement Plan is a long-term, organized plan that will be used by a community to address identified public health priorities.

2. What is an Action Plan?
The purpose of an action plan is to outline the specific tasks that need to be completed to reach a goal. There are measurable goals identified for each CHIP priority. The goals are further broken down into objectives with associated actions steps or strategies of how to accomplish the objectives.

Each of the Health Priorities has an Action Plan that can be view at the bottom of its respective page.

3. Why is the alignment of our priorities and goals with local, state, and national goals important?
When the efforts of local and state health improvement plans are aligned, the impact of each individual plan and effort is greater than if they had completely different focuses.

4. Are the Community Health Assessment and the Community Health Improvement Plan (CHIP) available to the public?
Yes! Your support and participation are important to the success of the Community Health Improvement Plan and we encourage you to get involved.

5. Where can I find a copy of the Community Health Assessment and the Community Health Improvement Plan (CHIP)?
An electronic copy for you to read or/and print can be found in both the “What’s New” and “General Resources” pages. You can also visit us at the Lincoln County Health Department for your own copy.

6. How can I stay informed about Healthy People Lincoln County and the Community Health Improvement Plan (CHIP)?
The best way to stay informed is to check our website. To join our mailing list and receive regular emails from us, please email Shelley Hersil at shersil@co.lincoln.wi.us.

7. How can I get actively involve?
Each health priority has a community coalition that activity works on strategies to address issue, oral health, nutrition, and mental health and substance abuse. To learn more on how you can join a coalition or volunteer at events contact Shelley Hersil at shersil@co.lincoln.wi.us.
